Step by step

Banned members DM the bot and fill in a form (default question: "Why were you banned?"; add your own). Appeals land in a review channel with Accept / Reject / Ask for info buttons and a cooldown between attempts. Decisions notify the member and are written to the moderation log; auto-unban lifts the ban on accept.
